Never. The SMTP check opens a connection, asks "does this mailbox exist?", reads the answer, and hangs up before any message is transmitted.
Some mail servers greylist or block verification probes. When that happens we return unknown instead of guessing — we'd rather under-flag than burn your list with false invalids.
